I dont know the ins and outs of it all, because fuck me who really does? But I can tell you that employers that offer those better healthcare services pay a portion of it. Often times how good/expensive your employer provided plan is depends on how much the employer is paying for on their end. If you have good benefits at a reasonable cost, thats likely your employer doing you a solid. Usually places that make good money and dont want high turnover.
